Report Problem Areas Form

Use this form when a patron reports an area in the stacks that is out of order or needs to be reorganized.

http://www.westga.edu/~library/depts/access/stacks/ -> select Report Problem Areas

• Enter the call number letters of the range(s) or area(s) that need to be corrected (ex: PS, BM, ML) into the Problem Area field. If the entire call number is available, please include that information also.
• If the patron reporting the problem area wishes to be notified once the problem is corrected, include their email address. Comments with specific details about the problem area are always helpful.

Click Submit and you're done!

An email will be sent to the patron via the email address on the form once the problem area has been corrected.
